Eswatini: The royal bet of a health system that keeps its promises
In Parliament, during the Speech from the Throne marking the opening of the third session of 12th Parliament, King Mswati III gave an unambiguous directive: the shortages of medicines that have plagued public health institutions for years must end, immediately. The monarch recalled that this issue, which weighs heavily on patient care as well as on public finances, was already among the main grievances expressed by the population during Sibaya in November 2023, but the government efforts undertaken since then have not been sufficient to put a lasting end to it.
The observation is harsh. A recent investigation found that a simple administrative error made in 2023 by the Ministry of Health, suspending, without a replacement device, the only agent authorized to authorize imports of controlled drugs was enough to cause nearly ten months of stock-outs on essential treatments, including for pain and mental health disorders. An independent audit commissioned by the authorities also highlighted irregularities in procurement procedures, as well as structural failures: non-payment of suppliers, weak procurement systems, and poor inventory management.
It is this weakened supply chain that the royal directive now intends to reform in depth, by requiring more robust mechanisms for the management, traceability and payment of pharmaceutical suppliers. The kingdom is also building on concrete progress in health coverage: the launch, in March 2026, of the national HIV prevention program integrating lenacapavir illustrates the capacity of the Eswatini health system to deploy major therapeutic innovations, when the political will and logistical means are gathered.
By placing this reform under his direct authority, King Mswati III intends to transform a recurrent popular grievance into an essential state priority, with a clear objective: to guarantee every Swazi, wherever he or she is in the kingdom, continuous access to essential care and medicines.
